    Integration contract update

    • Webhook HMAC uses only X-Meridova-Signature; X-CareStack-Signature is no longer accepted.
    • Outbound events use only X-Meridova-Event.
    • FHIR Patient identifiers use urn:meridova:national-id:<type> only (legacy urn:carestack:* removed).
    • Patient and staff identity is nationalIdType / nationalIdValue; the legacy cnic column is dropped (API still accepts cnic in request bodies).
